54-year old man who recently underwent a major cardiac procedure.&nbsp; At first glance, this ECG may not appear to be particularly unique but a closer inspection reveals something unusual going on here.&nbsp; <br><br><span style="text-decoration: underline;"><strong>HINT:</strong></span>&nbsp; The telling clue can be found in lead II but you'll have to use calipers and think "outside-the-box".&nbsp; 3 days after this ECG, the patient reverted back to sinus rhythm but continued to display this one finding.&nbsp; Would you call this NSR, since it is from a child?&nbsp; The pre-op heart rate in this child was 120/min.</p><p>For your more advanced students, remind them that, in adults especially, a heart rate close to 150/min. should cause them to examine the ECG in several leads, looking for the presence of atrial flutter with 2:1 conduction.&nbsp; Unfortunately, this ECG dates from a time period when our facility's computer database did not store complete 12-lead ECGs.&nbsp; The technology at the time was known as "reduced data", so much of the entire 12-lead ECG was deleted and only leads II, V1, and V5 were preserved for review by the cardiologist.&nbsp; This was in an attempt to reduce the amount of information that the computer would need to store over time.&nbsp; I can only imagine that this method was eventually deemed to be an undesirable way of storing ECG data.&nbsp;
